Service Description
Case Management Support • Case Managers will ensure that participants receive all necessary services and make progress in overcoming social barriers. Case Managers are responsible to make contact with clients once assessment has been completed. The Case Manager will have contact with clients all school year. *2 times a month or as needed based on the client’s assessment.
